*Job Summary*
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in the home health and/or hospice setting. The ideal candidate will possess extensive clinical experience, including familiarity with EMR/EHR systems such as Epic, Axxess, Therap. This role involves performing comprehensive assessments, administering medications and injections, case coordination with physicians offices and pharmacies, and supporting patients with diverse needs including geriatrics, and chronic conditions such as dementia and Alzheimer's. The LPN will work closely with interdisciplinary teams to ensure optimal patient outcomes in home care, or long-term care environments.
*Responsibilities*
* Conduct physical examinations and vital signs assessments to monitor patient health status.
* Administer medications, injections, and perform venipuncture with aseptic technique.
* Assist with medical procedures such as catheterization, tube feeding, wound suturing.
* Document patient care accurately using EMR/EHR systems while maintaining HIPAA compliance.
* Support discharge planning and case management activities to facilitate smooth transitions of care.
* Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, infectious diseases, and post-acute recovery.
* Collaborate with physicians and other healthcare professionals in settings such as Assisted Living Facilities, Rehab or Transitional Care units.
* Educate patients and families on health coaching topics including nutrition, medication management, and chronic disease management.
* Maintain infection control standards and sterile processing protocols during all procedures.
*Qualifications*
* Valid Licensed Practical Nurse (LPN) license in good standing.
* Proven experience working with EMR/EHR systems and technology.
* Experience across multiple healthcare environments including long-term care (nursing homes), assisted living facilities, hospice care, or home & community care.
* Knowledge of medical terminology related to cardiology (e.g., cardiac catheterization), behavioral health (dementia care), and infectious disease protocols.
* Proficiency in performing phlebotomy and administering injections with a focus on patient safety and comfort.
* Ability to work independently while collaborating effectively within multidisciplinary teams.
* Strong understanding of HIPAA regulations and medical documentation standards.
* Excellent communication skills with the ability to assess patient needs accurately and provide compassionate care for diverse populations including children with neonatal needs or adults requiring acute pain management.
