*Summary*
We are seeking a dedicated Pharmacist to join our team at AIDS Ministries. In this role, you will be instrumental in providing pharmaceutical care to our patients, ensuring safe and effective medication management while contributing to our mission of supporting the health of the community.
*Responsibilities*
* Dispense medications accurately and provide patient counseling on their proper use.
* Collaborate with healthcare providers to optimize patient medication regimens.
* Conduct medication therapy management and monitor patient outcomes.
* Maintain accurate records of prescriptions and patient interactions.
* Educate patients about medication side effects and interactions.
* Participate in community outreach programs to promote health education.
*Requirements*
* Pharmacy degree from an accredited institution.
* Active pharmacist license in the state of Indiana
* Strong knowledge of medical terminology and pharmacy software applications.
* Excellent communication skills with a focus on patient care and service.
* Experience with 340B and working with high need community members.
