*Job Title: *Physical Therapist

*Pay*: Negotiable

*Position Summary: *Responsible for performing initial evaluations and administer treatments to patients using various physical therapy modalities and procedures based upon physicians ordered and within the practices standards of LOG. Must accurately document patient responses to the treatment and completes proper and timely charges.

*Qualifications:*

* Must be a graduate of an accredited, approved physical therapy program and be a licensed Physical Therapist or eligible for licensure in the state of Pennsylvania.
* CPR certification (required)
* Valid PA Drivers License (required)
* Valid PA Therapist License (required)
* Membership with the APTA and abides by the APTA Standards of Practice (preferred)
* Display knowledge of the use of various Exercise equipment, PC, copier, fax, and telephone (required)

*Essential Duties:*

* Performs initial evaluations and establishes appropriate long and short term measurable goals. Provides appropriate treatment following clinic procedures and established techniques.
* Accurately records patient response to the treatment each time a patient is treated. Submits charge forms to the clerical department which accurately reflect the treatment provided to each patient including time in and time out. The total number of unites billed shall not be greater than total number of time calculated using the time-in and time-out.
* Strives to maintain open, effective, professional communication and a positive working relationship with the Clinic Manager and all staff.
* Maintains the highest level of patient confidentiality.
* Must possess flexibility in their schedule to ensure coverage and accommodate patient's needs.
* Demonstrates a positive attitude toward the profession and the organization. Strives for strong customer service continuously. Must have a strong professional demeanor, possess effective interpersonal skills and demonstrates a positive attitude that promotes cooperation to administrations, physicians, staff members, patients and other professionals such as work rehab nurses and vendors.

Job Type: Full-time

Pay: $70,000.00 - $95,000.00 per year
