A reputable and growing home care agency is seeking a compassionate and motivated Field Registered Nurse to provide high-quality clinical care to patients in their homes. This role offers autonomy, meaningful 1:1 patient interaction, and the opportunity to make a direct impact on patient health and independence.
*Key Responsibilities*
* Provide skilled nursing visits in the patient's home, including assessments, wound care, medication management, and disease education
* Develop and update individualized patient care plans based on clinical findings and physician orders
* Document visits accurately and in a timely manner according to regulatory and agency standards
* Teach and support patients and families on health conditions, medications, and care techniques
* Communicate regularly with physicians, caregivers, and the interdisciplinary team to ensure continuity of care
* Monitor patient progress and promptly report changes in condition
* Participate in case conferences and quality improvement activities as needed
*Requirements*
* Active RN license in New York
* Experience in home care, medical-surgical nursing, hospital bedside nursing, or related setting preferred
* Strong clinical assessment skills and comfort working independently in the field
* Reliable transportation and ability to travel locally for home visits
* Excellent communication, time management, and documentation skills
