*Position Overview *

The *Staff Pharmacist *must abide by various regulatory compliance rules and must always treat patients and other staff members with dignity and respect. The Pharmacist will dispense medications, counsel patients effectively, complete MTM responsibilities, and give vaccinations. As this is a closed door, 340B retail and specialty pharmacy, the Pharmacist will work with the providers, medical assistants, and nurses at Five Rivers to ensure the patients' medication needs are meet.

*General Responsibilities*

* Dispenses all pharmaceuticals accurately as ordered, checks prescriptions filled by pharmacy staff for accuracy, performs drug utilization reviews for safety and effectiveness
* Ensures effective team functioning and quality patient care
* Counsels patients in accordance with laws and regulations, regularly identifies opportunities for patient education or intervention
* Provides information upon request to practitioners and staff members regarding pharmaceuticals, warnings, dosages, indications, incompatibilities or interactions, contraindications, and availability
* Maintains inventory of pharmaceuticals and other related supplies, including a system of recordkeeping for the purchase, sale, delivery, possession, storage, and safekeeping of such

*Qualifications*

* Graduate of an ACPE Accredited College of Pharmacy (BS in Pharmacy, PharmD)
* Current Ohio Pharmacy License in good standing
* 3+ years pharmacy experience in community or ambulatory care setting preferred
* Experience with the Functions of a 340B preferred
