*Position Summary*

FYZICAL Therapy & Balance Centers of Illinois is seeking a highly motivated and compassionate Physical Therapist to join our dynamic, patient-centered team. The Physical Therapist is responsible for evaluating, developing, and implementing customized treatment programs to help patients restore function, improve mobility, reduce pain, and prevent injury. This position requires maintaining a full caseload of approximately 10-12 patients per day, ensuring high-quality care and exceptional patient experiences consistent with FYZICAL's standards of clinical excellence.

*Key Responsibilities*

*Clinical Care*

* Conduct comprehensive evaluations of patients' physical abilities, movement patterns, and functional limitations.
* Develop individualized treatment plans based on physician referrals, evaluation findings, and patient goals.
* Deliver one-on-one, hands-on physical therapy sessions focused on optimal outcomes in strength, balance, mobility, and pain relief.
* Modify treatment plans as necessary based on patient progress and ongoing assessment.
* Maintain detailed and accurate documentation in compliance with Illinois Department of Financial and Professional Regulation (IDFPR) standards, Medicare, APTA, and payer-specific guidelines.
* Educate patients and caregivers on home exercise programs, self-management strategies, and injury prevention.
* Manage and sustain a daily caseload of and average of at least 10-12 patients, balancing efficiency with quality of care.
* Collaborate with referring physicians, physical therapist assistants, and clinical support staff to ensure coordinated, patient-focused care.

*Professional Development & Compliance*

* Maintain an active Illinois Physical Therapist License in good standing.
* Stay current with continuing education requirements as defined by the IDFPR and professional practice standards.
* Participate in FYZICAL's ongoing clinical education programs and mentorship opportunities.
* Adhere to all applicable federal, state, and local laws, including HIPAA and IDFPR compliance regulations.
* Demonstrate professional conduct, integrity, and ethical standards at all times.
* Complete training and accountability consistent with FYZICAL Chicago's Therapist training manual.

*Clinic Operations & Team Contribution*

* Support clinic growth and success by promoting positive patient outcomes and word-of-mouth referrals.
* Participate in staff meetings, community outreach, and marketing initiatives as requested by clinic leadership.
* Contribute to a collaborative team environment that emphasizes communication, accountability, and respect.
* Assist with training and mentorship of new clinicians and students as needed.
* Promote FYZICAL's mission of improving lives through innovative therapy, balance training, and fall prevention programs.

*Accompany Sales Liaison to physician's offices and meetings to network services and marketing efforts, as assigned.*

* Maintain an average daily caseload of 10-12 patients.
* Deliver consistent, measurable improvements in patient outcomes and satisfaction.
* Complete all documentation accurately and on time per clinic policy.
* Uphold FYZICAL's standards of quality, professionalism, and clinical excellence.

*Physical Job Demands*

* Ability to stand, walk, and move frequently throughout the workday.
* Ability to lift and carry up to 50 pounds regularly and assist patients with mobility, transfers, and exercise activities.
* Frequent reaching, bending, and manual manipulation associated with patient treatment.
* Visual and auditory ability to observe and evaluate patients effectively.
* Capacity to maintain prolonged focus during clinical and administrative tasks.
* Ability to safely demonstrate exercises, balance techniques, and equipment use as required.
* Must be physically capable of performing all essential job duties with or without reasonable accommodation.

*Qualifications*

* Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) from an accredited program, or equivalent.
* Current, unrestricted Illinois Physical Therapist License (or eligibility to obtain prior to employment).
* Current CPR/BLS certification.
* Strong clinical reasoning, manual therapy, and communication skills.
* Excellent time management and organizational abilities to sustain a full caseload.
* Experience or interest in orthopedic, vestibular, balance, and neurological rehabilitation strongly preferred.
* Commitment to ongoing professional growth and patient-centered care.

Pay: $82,500.00 - $100,000.00 per year
