*Registered Nurse - Hospice CareJob Summary*
The Registered Nurse (RN) plans, organizes, and delivers high-quality hospice care. With a strong foundation in nursing and an emphasis on community health, the RN provides compassionate care that supports both patients and their families in home and community settings. This role builds upon community resources and works closely with the interdisciplinary team to meet each patient's unique physical, emotional, and spiritual needs.
*Essential Job Functions & ResponsibilitiesPatient Care*
* Conducts an initial and ongoing comprehensive assessment of patients and families to determine hospice care needs. Includes a thorough physical assessment and medical history review.
* Provides skilled nursing care using the full scope of the nursing process.
* Continuously assesses and evaluates patient status by:
* Developing and initiating individualized plans of care
* Regularly reassessing the needs of patients and caregivers
* Updating the plan of care as needed in collaboration with the care team
* Adjusts care plans based on changes in patient condition and evolving needs.
* Uses health assessment data to determine appropriate nursing diagnoses.
* Establishes care goals based on nursing diagnoses, incorporating palliative interventions, and involving both patients and families in the planning process.
* Administers prescribed medications and treatments, and initiates preventive and rehabilitative nursing procedures.
* Provides education and counseling to patients and families to support informed decision-making and symptom management.
* Offers instruction on health care practices and self-care techniques tailored to each patient's condition.
* Assists with activities of daily living, enhancing patient comfort, dignity, and independence.
* May serve as Case Manager, coordinating the care of an assigned caseload in collaboration with the clinical supervisor.
*Communication & Collaboration*
* Accurately documents clinical notes, including assessments, interventions, and patient responses, with special attention to pain and symptom management.
* Communicates proactively with physicians regarding changes in patient condition and care needs; obtains and implements physician orders promptly.
* Coordinates with community healthcare providers and services to ensure continuity of care.
* Provides thorough education to patients and caregivers, including medication management, dietary guidelines, and safety protocols.
* Maintains a safe and supportive environment for patients and their families.
* Collaborates with interdisciplinary team members to support emotional, psychosocial, and spiritual aspects of care.
* Participates in regular interdisciplinary team meetings to review patient progress and refine care plans.
*Additional Responsibilities*
* Participates in on-call rotation according to agency policies.
* Ensures timely coordination of necessary equipment, supplies, and support services.
* Supervises hospice aides and other assigned staff, delegating appropriately.
* Maintains professional knowledge and competence through continuing education, certifications, and in-service training.
* Accepts case assignments as requested and follows through on responsibilities.
* Actively engages in Quality Assurance and Performance Improvement (QAPI) initiatives.
*Qualifications*
* Graduate of a National League for Nursing (NLN) accredited school of nursing.
* Current and active Registered Nurse (RN) license in the state of practice.
* Must meet one of the following:
* Minimum of one (1) year of professional nursing experience within the past three (3) years, *or*
* Bachelor of Science in Nursing (BSN) from an NLN-accredited program with current licensure.
* Preferred: Two (2) years of nursing experience, including at least one (1) year in hospice, home health, or community/public health nursing.
* Management experience is not required; however, the RN must be capable of supervising hospice aides and ancillary personnel as necessary.
