*Position Summary:* The Hospice RN Case Manager coordinates and delivers comprehensive nursing care for patients and families facing terminal illness. This role is central to the interdisciplinary hospice team, ensuring that each patient receives individualized, compassionate, and high-quality end-of-life care. The RN Case Manager oversees clinical care plans, provides direct patient visits, and serves as the primary point of contact for patients, families, and caregivers throughout the hospice experience.
*Duties and Responsibilities: *
* Serve as the primary case manager for an assigned group of hospice patients, coordinating the plan of care across disciplines.
* Perform comprehensive assessments to identify physical, emotional, social, and spiritual needs of patients and families.
* Develop, implement, and updateindividualized plans of care in collaboration with the interdisciplinary team (IDT).
* Provide skilled nursing interventions, symptom management, and patient education in accordance with physician orders and hospice protocols.
* Facilitate effective communication among physicians, team members, patients, and caregivers to ensure continuity of care.
* Conduct regular home and facility visits to monitor condition changes, evaluate treatment effectiveness, and adjust care plans accordingly.
* Provide emotional support, counseling, and education to patients and families regarding disease progression, comfort measures, and end-of-life expectations
* Maintain accurate and timely documentation that meets hospice regulatory and compliance standards.
* Participate in interdisciplinary team meetings, case conferences, and quality improvement activities.
* Support patients and families through crisis management, emergent symptom control, and coordination of after-hours needs with on-call staff.
* Ensure safe medication management, equipment usage, and adherence to infection control practices.
