Star Home Health is committed to providing exceptional, patient-centered care in the comfort of our clients' homes. We are currently seeking a dedicated and compassionate *Speech Therapist (SLP)* to join our growing clinical team.

*Position Summary:*

The Speech Therapist provides skilled speech therapy services to home health patients, including evaluation, treatment planning, and ongoing therapy to improve communication, swallowing functions, and overall quality of life.

*Key Responsibilities:*

* Conduct initial assessments and develop individualized speech therapy care plans
* Provide speech, language, cognitive-communication, and swallowing therapy
* Prepare timely and accurate clinical documentation, including evaluations, progress notes, and discharge summaries
* Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care
* Educate patients and caregivers on therapy goals, home exercises, and safety practices
* Participate in case conferences as needed
* Adjust treatment plans based on patient progress and clinical findings

*Qualifications:*

* *Active Arizona Speech-Language Pathologist (SLP) license - Required*
* Master's Degree in Speech-Language Pathology
* Home health experience preferred but not required
* Strong clinical assessment and communication skills
* Ability to work independently and manage a flexible caseload
* Reliable transportation and valid driver's license
