We have an immediate opening for a *full-time and part-time Home Health Occupational Therapist in Salem, OR. *
*Duties & Responsibilities:*
* Provide rehabilitative and skilled OT services to patients in their place of residence.
* Performs manual and functional therapy exercises and activities with the patient.
* Complete safety assessments, and provide recommendations to interdisciplinary team, family, and caregivers
* Makes initial evaluation visits to patients and develops/implements an appropriate Plan of Care based on skilled assessment and various tests to determine patient's individualized needs.
* Provides instructions for patient, family members and other caregivers to ensure maximum opportunity for improvement.
* Assists in the supervision of CNAs and COTAs
* Completes Point of Care Documentation
* Meets agency productivity expectations
*Job requirements:*
* Graduate of a ACOTE accredited occupational therapy program
* Must possess a current Oregon OT license
* Current CPR certification
* Current & Valid OR Driver's License and Car Insurance
* Previous Home Health experience preferred
* Ability to work independently without supervision, excellent critical-thinking skills
* Ability to function as a member of the Interdisciplinary team, communicating at least weekly.
* Must be able to pass a criminal background check.
