Welcome Homecare is a 5 star agency that has been serving the community of the greater Jacksonville area for more than 25 years. WHC is seeking a Physical Therapist for our team in Jacksonville. We are interviewing for a Full-time position. Our PTs work under the order of the physician to provide skilled physical therapy and health teaching to patients and their caregivers in the home setting, as a team in conjunction with PTAs.
Responsibilities:
* Establishes physical therapy treatment plans to the patients as defined by the plan of care as prescribed by the physician.
* Supervises and works in conjunction with Physical Therapy Assistants.
* Observing, recording activities and findings in the clinical record and reporting to the case manager and/or physician the patient's reaction to treatment and any changes in the patient's condition.
* Collaborate appropriately with patients, caregivers, physicians, other therapists, nurses, aides, and other company staff on a regular basis based on the needs of the patient.
* Attends case conferences on a weekly basis.
* Performs discharge planning in conjunction with PTAs.
* Accurately and thoroughly document patient care observations, interventions and evaluation on the day services are rendered.
* Maintain and submit documents as required by the Company and/or facility.
* Rotates On-Call weekends as census and need indicates
Qualifications:
* Must hold valid, clear, current licensure as a Registered Physical Therapist in the State of Florida
* Home Health experience highly desired
* One year prior experience required.
* Prior experience with electronic medical records preferred.
* Current CPR certification is required.
* Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
