*Position Overview: *
Advanced Hospice is seeking an experienced and resilient RN Clinical Liaison to join our fast-growing team. This role is essential in providing high-quality hospice care by ensuring seamless communication and education among patients, families, and healthcare facilities.
*Location: *
Pittsburgh, PA
*Schedule: *
Monday to Friday with some rotating on-call responsibilities.
*Travel Requirements: *
This position requires 100% travel to patient homes, facilities, and hospitals as necessary.
*Responsibilities: *
* Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.
* Educate patients, families, and facility staff about hospice services and patient-specific care plans.
* Act as a primary point of contact to ensure coordination and continuity of care.
* Utilize strong communication and time management skills to effectively manage multiple cases and priorities.
* Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.
* Provide ongoing support and education on the hospice process to healthcare providers within the community.
*Qualifications: *
* Must possess an active RN license.
* Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.
* Exceptional interpersonal, communication, and presentation skills.
* Robust organizational and time management skills are essential.
* Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.
