*Duties & Responsibilities*
* Provide rehabilitative and skilled PT services to patients in their homes under the supervision of a licensed Physical Therapist
* Perform manual therapy and functional exercises with patients
* Report changes in patient condition to the supervising PT and interdisciplinary team
* Assist in the supervision of CNAs
* Utilize various assessments to identify patient needs and adjust the plan of care under PT direction
* Complete accurate and timely point-of-care documentation
* Meet agency productivity expectations
*Job Requirements*
* Graduate of an accredited Physical Therapy Assistant program
* Current Oregon PTA license
* Valid Oregon driver's license
* Valid auto insurance
* Current CPR certification
* Home health experience preferred
