*Crescent Manor Nursing and Rehabilitation Center, located in scenic Bennington, VT, *is actively seeking *LPN's *for our skilled nursing facility. We are looking for reliable nurses to provide quality care to our residents. The ideal candidates will be qualified, compassionate, delivering services in a patient and calm manner in a dynamic environment.

*DUTIES:*

* Administer medications and treatments timely and as ordered
* Provide supervision and guidance to LNA's
* Monitor patients' and residents' condition
* Assist patients and residents as necessary with ADL's
* Responsible for the accurate and timely completion of medical records and documentation
* Communicate with patients' and residents' family or friends as required
* Collect samples for lab as ordered
* Maintain flexibility with work schedule to address unpredictable needs

*REQUIREMENTS:*

* Current VT LPN license required
* Ability to work in a team environment
* Ability to multi-task
* Excellent communication and interpersonal skills
* Understanding of confidentiality obligations and best nursing practices
* Competent in EMR entry - experience with Point Click Care a plus

Ability to meet the physical requirements of the position

Crescent Manor houses a specialty Dementia Unit and a specialty Huntington's Disease unit.
