*Who We're Looking For*
* Licensed Vocational Nurse (LVN) in the State of Texas
* Prior wound care experience a plus (but training provided if you're new)
* Compassionate, dependable, and committed to ethical patient care
* Enjoys hands-on nursing with minimal red tape
_*Responsibilities/Duties*_
* Furnishes skilled nursing services as assigned an in accordance the plan of care.
* Prepares clinical and progress notes and submits to the agency according to agency protocols.
* Accurately and effectively documents skilled observations and assessments, treatments and medications administered, patient/caregiver teaching, patients/caregivers response, and communication with physician and RN of findings and follow-up as appropriate.
* Reports changes in patient status to RN and physician.
* Updates the medication list with changes
* Documents physician orders and submits to RN for review and implementation of the change in plan of care.
* Assist the physician orders and registered nurse in performing specialized procedures in accordance with the plan of care, LVN scope of practice, and individual competency.
* Prepares equipment and materials for treatments observing aseptic technique as required to implement the plan of care.
* Assist the patient in the learning appropriate self-care techniques.
* Participates in patient case conferences and staff meetings as directed.
* Attends in-service and continuing education programs as requested.
* Assists with orientation of new personnel as requested.
* Participates in QAPI activities as requested.
* Drives to and from patient residences to perform duties
* Performs other agency-related duties as assigned.

Pay: $55,500.00 - $75,400.00 per year
