*Overview*
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in patient care, particularly with individuals who are recovering from injury, managing chronic conditions, or facing physical and cognitive challenges that impact daily functioning. This role involves working closely with patients to develop personalized treatment plans aimed at improving their daily living skills and overall quality of life. The Occupational Therapist will collaborate with multidisciplinary teams in various settings, including nursing, nursing schools and support staff.
*Responsibilities*
* Assess patients' needs and develop individualized treatment plans to enhance their ability to perform Activities of Daily Living (ADLs).
* Provide therapeutic interventions for patients with disabilities and other challenges.
* Collaborate with educators and healthcare professionals in school settings to support students' occupational therapy goals.
* Implement discharge planning processes to ensure smooth transitions for patients moving from one care setting to another.
* Maintain accurate documentation of patient progress and treatment outcomes using medical terminology.
* Educate patients and their families on therapeutic techniques and strategies to facilitate recovery and independence.
* Participate in team meetings and contribute insights regarding patient care strategies in post acute care environments.
*Experience*
* A degree in Occupational Therapy from an accredited program.
* Valid state licensure or certification as an Occupational Therapist.
* Experience working with patients in post acute settings is preferred, along with familiarity in addressing the needs of individuals looking for recovery.
* Knowledge of discharge planning processes and experience in post acute care settings is advantageous.
* Proficiency in medical terminology relevant to occupational therapy practices.
