RN/LPN On-Call Position

Responsibilities:

* Provides emergency, PRN, routine, and admission visits as necessary or scheduled.
* Assumes primary responsibility for hospice patients during on-call hours.
* Provides ongoing assessments of physical, psychological, and social and spiritual needs of the hospice patient and family and initiates appropriate interventions.
* Assesses and evaluates patient's status by following the plan of care, regularly reevaluating patient, and family/caregiver needs, participating in revising the plan of care as necessary based on patient status and needs.
* Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing interventions/actions. Includes the patient and the family in the planning process.

Qualifications:

* Hold a current unencumbered license as a RN or LPN.
* Hospice health experience preferred but not required.
* Must be computer proficient in typing and various programs, including background in EMR.
* Possess and maintains current CPR certification.
* Must be flexible to adjust to changing tasks frequently.
* Must have a valid driver's license, reliable vehicle, maintain valid auto liability insurance, and be able to drive throughout service area under a variety of weather conditions.
* Excellent verbal and written communication skills.
