Primary Care APP – Team Member Clinic

Augusta Health is launching a dedicated Team Member Clinic to provide exceptional care to the people who care for our community. We’re seeking an experienced Nurse Practitioner or Physician Assistant to join this collaborative, mission-driven setting. In this role, you’ll provide comprehensive outpatient care—including wellness visits, chronic disease management, and same-day acute care—while working closely with physicians, nurses, health coaches, and administrative staff. You’ll engage in care coordination, patient education, and team huddles, all supported by a streamlined clinical and administrative infrastructure to ensure high-quality, efficient care delivery.

Practice Details

• Schedule: Monday–Friday, approx. 9 AM–5:30 PM

• Patients: 14–16 per day (mix of scheduled and acute visits)

• Call: Shared with the Primary Care service line; planned annually
