Shoreland Family Medicine, a well-established primary care practice within Mercy Health Toledo, is seeking a Board-Certified Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) or Family Medicine Physician to join our collaborative team. This opportunity is ideal for an experienced provider, though motivated new graduates are encouraged to apply.

Practice Highlights:

  • Monday–Friday schedule, 8:00 a.m. – 5:00 p.m. supporting work-life balance
  • Shared practice call with a supportive provider group
  • Immediate access to an established patient panel due to practice growth and panel absorption
  • Dedicated mentorship and clinical support from Dr. Subramanian and Dr. Hanna
  • Collegial, team-oriented environment focused on high-quality, patient-centered care

Responsibilities:

  • Provide comprehensive primary care services across the lifespan, including preventive care, chronic disease management, and acute visits
  • Develop lasting relationships with patients and families while promoting wellness and continuity of care
  • Collaborate with physicians, advanced practice providers, nursing staff, and care coordinators to deliver evidence-based care
  • Utilize EPIC EMR to support efficient, coordinated clinical workflows
  • Participate in quality initiatives and population health strategies

Qualifications:

  • Board Certification required (Family Medicine for physicians; AANP/ANCC or NCCPA for APCs)
  • Active Ohio license or eligibility to obtain licensure prior to start
  • Strong clinical judgment with a patient-first mindset
  • Excellent communication and teamwork skills
