NP/PA - Family Practice Black Creek

ThedaCare Physicians is seeking an NP/PA Family Medicine APC who has an interest in supporting rural clinics with a strong team of providers and a great support staff for our Black Creek location.

Position Details:

• 1.0 FTE
• Number of Physicians in this group (Appleton East/West and Black Creek): 18
• Number of NP/PA Providers in this group (Appleton East/West and Black Creek): 10
• Providers in Black Creek: 1 MD and 1 NP
• 4 workdays with 36 patient contact hours (Possibly 1-3 weekends per year)
• No Call
• 15-20 Patients per day
• MA support would be assigned, RN and PSR support for the team
• Experienced APC preferred but will accept new graduates

Small Town Friendly with Big City "Like" Facility:

ThedaCare Physicians Black Creek is a 2,600 sq-ft facility with a fully integrated EMR and digital PACS system to view radiologic films. The facility has 4 exam rooms, 1 procedure room, as well as a central nursing station and dedicated physician office.

Practice Opportunities:

The current physician, who is relocating within the organization, performs minor office procedures and sees newborns at the local hospital.  Leadership and volunteer opportunities are available within ThedaCare and the local community, including but not limited to: disease management,  volunteering as the attending at local family medicine residency program, precepting medical and nurse practitioner students, medical directorship with local schools, and providing local nursing home coverage .

There are 3 major shopping malls, including the famous Fox River Mall in Appleton.
