Highlights
Community Physician Network seeks an experienced Adult/Geriatric Nurse Practitioner to join our Care Planning Partners program located on the north side of Indianapolis. The goal of this program is to provide high level geriatric/palliative care expertise and support within several primary care practices. Services include early identification of geriatric syndromes and symptom management needs as well as a focus on advance care planning.

 

  • Interdisciplinary team approach with primary care, geriatrics, and palliative care providers, as well as nursing support staff
  • Located within an established Community primary care practice on the campus of Community Hospital North, with potential to travel to other offices on Indianapolis’ north side
  • Monday-Friday (8am – 5pm); no call or weekend commitment
  • EPIC EMR

  • Qualifications
  • Ideal candidates will have demonstrated expertise in the advanced practice of nursing within the Adult/Geriatric population and an understanding of the specific needs of this population.
  • Must have the desire to work both independently and as part of a multi-disciplinary team.
  • Must have experience with advance care planning
  • Flexibility, “go-getter” mentality, strong communication, and interpersonal skills are essential


Licensure & Certification
Educational background consistent with requirements for licensure and certification within the state of Indiana.
