The Nutritionist/Dietitian provides medical nutrition therapy, develops personalized dietary plans, and educates clients on healthy eating habits for *Medicaid populations, including individuals with multiple chronic illnesses, mental health conditions, substance use disorders, and pregnant mothers.* The ideal candidate will have at least *five years of experience* delivering nutrition interventions in community-based, clinical, or social service settings. This role requires a compassionate, patient-centered approach to support vulnerable populations in achieving better health outcomes through tailored nutrition plans and education. We are seeking candidates that are bi-lingual in: Spanish


* Conduct *comprehensive nutritional assessments* for individuals with complex medical and social needs.
* Develop and implement personalized nutrition care plans addressing chronic disease management, mental health conditions, prenatal nutrition, and substance use recovery.
* Collaborate with care coordinators, social workers, and healthcare teams to ensure integrated support for clients.
* Provide nutrition education and counseling for clients and their families, promoting sustainable dietary habits.
* Monitor and track clinical outcomes related to diet and lifestyle interventions, adjusting plans as needed.
* Support Medicaid clients in accessing nutritional resources, food benefits (SNAP, WIC), and meal programs.
* Maintain accurate and timely documentation of client interactions following the program standards.
* Participate in community outreach, workshops, and group education sessions to promote public health initiatives.
* Stay updated on evidence-based nutritional practices for populations with chronic disease, mental health needs, and substance use disorders.

Job Types: Full-time, Per diem

Pay: $38.31 - $41.05 per hour
