We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will be responsible for assessing, planning, and implementing personalized treatment programs for patients with various physical impairments. This role requires a strong understanding of anatomy and physiology, as well as the ability to work collaboratively with other healthcare professionals to ensure optimal patient care. The Physical Therapist will work in various settings including inpatient, outpatient, and home health environments.

*Responsibilities*

* Conduct thorough evaluations of patients' physical conditions and develop individualized care plans.
* Implement physiotherapy treatments based on established care plans, focusing on improving mobility and functionality.
* Provide discharge planning and ensure continuity of care for patients transitioning from acute care settings.
* Collaborate with multidisciplinary teams to coordinate patient care and optimize treatment outcomes.
* Maintain accurate documentation of patient progress using EMR systems.
* Educate patients and their families about treatment plans, exercises, and self-care strategies.
* Utilize various therapeutic techniques including trigger point therapy, orthopedics, and geriatrics to enhance recovery.
* Monitor patient progress and adjust treatment plans as necessary to meet individual needs.

*Skills*

* Strong knowledge of medical terminology and anatomy relevant to physical therapy.
* Experience in acute care, inpatient, outpatient, pediatrics, geriatrics, home health, and clinic environments.
* Proficient in developing comprehensive care plans tailored to individual patient needs.
* Familiarity with EMR systems for effective documentation and tracking of patient progress.
* Excellent communication skills for effective interaction with patients, families, and healthcare team members.
* Ability to apply physiotherapy techniques effectively while demonstrating empathy and compassion towards patients.
* Understanding of home & community care practices to support patient rehabilitation in their daily environments. Pay: $50.00 - $75.00 per hour

Expected hours: 3 - 20 per week
