*Overview*
We are seeking a highly motivated clinician to join our team to care for patients in our brand new dialysis clinic opening later this fall. In addition to hemodialysis patients, there will be some opportunity to care for peritoneal dialysis patients.
*Duties*
* Conduct comprehensive patient assessments to evaluate dietary needs and nutritional status.
* Develop and implement individualized nutrition care plans tailored to each patient's specific health conditions.
* Collaborate with the endocrinology team to teach lifestyle modification, diabetes self management, and healthy nutrition habits to improve patient quality of life.
* Monitor and assess the effectiveness of nutrition interventions and blood glucose management, making adjustments as necessary.
* Generate reports regarding patient attendance, compliance, and outcomes related to services provided.
*Requirements*
* Bachelor's degree in Dietetics from an accredited institution.
* Registered Dietitian Nutritionist (RDN) credential and state licensure is required.
* Experience in hospital settings or long-term care facilities is preferred.
* Strong skills in patient assessment and the ability to develop effective nutrition plans.
* Knowledge of renal nutrition needs and challenges associated with health literacy and compliance.
* Familiarity with blood glucose management, nutrition and diet impact on biochemistry and lab values, as well as adaptable education techniques.
* Excellent communication skills, both verbal and written, with the ability to educate diverse patient populations.
* Ability to work collaboratively within a multidisciplinary team environment.
* Passion for promoting health through nutrition education.
*Preferred Qualifications*
* Experience working with CKD and ESRD
* CRS credential preferred
