*Job Title: Behavioral Health Therapist*
*Department: Provider*
*Schedule: *hybrid in-office and remote; 32-40 hrs./wk.; fixed schedule; primarily between 8 am-5 pm Monday-Friday; some flexibility for consistent, remote evenings or weekends for interested providers in collaboration w/Behavioral Health Manager

*Job Summary:* Provides diagnostic assessment, treatment planning, and outpatient therapy in both Spanish and English to patients of identified age groups with a wide range of mental health conditions. Works with the Medical Director and a health care team, including MDs, NPs, RNs, PAs, CNMs, MAs, and LADCs. Independent responsibility and accountability in decision-making. Provides individual, couples, family, or group therapies as appropriate based on provider experience & capacity.

*Essential Functions*:
* Perform diagnostic assessment & treatment planning guided by the DSM-5 & Minnesota DHS standard
* Provide ongoing therapy to clients with a variety of presenting concerns, including mental health, substance use, and co-occurring disorders from a harm-reduction, client-centered perspective
* Maintain thorough & timely documentation & billing of services
* Collaborate & consult with other NHS providers, staff, & members of the client system patients as appropriate with client-centered & integrated behavioral health model.
*Qualifications:*
* Active Minnesota license as an LPCC, LMFT, LICSW, or LP.
* Master's degree or higher in psychology, counseling, social work, or marriage and family therapy.
* Minimum of two years of direct client care, with primary care setting experience preferred.
* Proficiency with using computer hardware and software for clinical documentation and Microsoft Office applications.
* Proficiency in assessment & diagnosis, treatment planning, risk assessment, intervention, and care coordination in mental health.
* Intent & commitment to practice through trauma-informed, decolonial, & intersectional lenses.
* Specific familiarity with IFS, DBT, ACT, EMDR, narrative, brainspotting, and other models is a plus.
* Fluency in English & Spanish to provide the full range of assessment, treatment planning, and therapy services.
