AxiomHealth Management is seeking Certified Lymphedema Therapists (CLTs) to join our PRN / Per Diem 1099 Independent Contractor pool. This role offers flexible scheduling, per-visit pay, and the opportunity to provide high-quality lymphedema care in the home health setting.
*Pay (1099 - Flat Rate)*
* *$120 per evaluation / reassessment*
* *$90 per treatment visit*
*Schedule*
* PRN / Per Diem
* No minimum visits or fixed schedule
* Accept assignments based on your availability
* Visit frequency and time in the home must follow the patient's plan of care
*Responsibilities*
* Perform lymphedema evaluations, reassessments, and treatment visits in patients' homes
* Deliver evidence-based lymphedema care per physician orders and plan of care
* Complete timely and accurate clinical documentation
* Communicate patient progress and clinical findings as needed to support continuity of care
* Coordinate care with other disciplines involved in the patient's treatment
* Maintain compliance with professional standards, infection control practices, and regulatory requirements
*Qualifications*
* Active Florida license as PT, OT, PTA, or COTA
* Certified Lymphedema Therapist (CLT) (LANA-recognized program required)
* 1+ year of home health experience (lymphedema experience preferred)
* Valid driver's license and reliable transportation
* Strong documentation and computer skills
* Bilingual skills a plus
*Independent Contractor (1099)*
* This is a true 1099 independent contractor position
* Contractors are responsible for their own taxes, insurance, and business expenses
* No uniforms or equipment provided; clinicians must supply their own equipment, scrubs or professional attire
* Patient supplies will be provided.
*Physical Requirements*
* Ability to lift at least 30 lbs
* Ability to work from low surfaces, bend, stoop, squat during patient care
