Overview - Palliative Care
Collaborate with 1-3 physicians and 3 advanced practice providers, case manager, hospice coordinator, and Pastoral Care Team members
Responsibilities include assessing a variety of symptom management cases, promoting communication and collaboration of multiple disciplines for end-of-life care and providing passionate care for patients with chronic and life-limiting conditions.
