CentraCare is hiring a full-time Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our team at the Melrose clinic.
Responsibilities
As a provider developing your own patient panel, you will independently see patients of all ages for acute and chronic care needs. Our CentraCare – Melrose clinic providers diagnose and treat short-term illnesses; provide preventive care including routine checkups, health-risk assessments, immunizations and screening tests; offer personalized counseling on maintaining a healthy lifestyle; and manage and coordinate care for those with chronic illnesses. The Melrose clinic has access to lab and imaging services during regular clinic as well as extended hours.
Schedule
- .9 – 1.0 FTE
- Option of a four-day work week
- Limited weekend work consisting of Saturday morning clinic rotation 1:6
- No call
- No holidays
Requirements
To be eligible for this position you must meet the following requirements:
- Certified as a Nurse Practitioner or hold a current Minnesota Physician Assistant Licensure and be eligible for Minnesota licensure.
- All applicants must have a current American Heart Association BLS Healthcare Provider card and Drug Enforcement Agency (DEA) registration number prior to receiving clinical privileges.

It is preferred that applicants have relevant practicing provider experience in one of the following areas:
- Family Medicine
- Emergency Medicine
- Urgent Care
Pay and Benefits
- Starting pay begins at $107,099.20 per year; exact wage determined by years of related experience
- Salary range: $107,099.20 - $122,449.60 per year
- Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
