Psychiatric Mental Health Nurse Practitioner - Primary Care Network - Saratoga Springs, New York
Compensation Range: $124K - $164K/Yr. (Compensation is tiered, based on years of experience.)
Join Saratoga Hospital Medical Group - Behavioral Health, as a Psychiatric Nurse Practitioner in a new position working within our primary care network, at Saratoga Hospital Medical Group Primary Care - Milton, after an initial period of orientation working with one of our Psychiatrists at our Wilton Primary Care location.
As a Psychiatric Nurse Practitioner with our group, you will provide psychiatric services to referred patients within our primary care network, performing behavioral health evaluations and treatment. As a member of our multidisciplinary team, work in close collaboration with Psychiatrists, Primary Care Physicians and Advanced Practice Providers, Behavioral Health Clinicians, Care Managers, and other clinical/administrative support staff.
Under medical direction of a collaborating physician, you will perform mental health assessments, manage the psychiatric care of patients, and provide health education to patients and their families, as well as other members of the clinical team.
All physicians and nurse practitioners take inpatient call for a 16-bed inpatient adult mental health unit. Call is paid and approximately 1:10, taken evenings by phone with rounding on weekends.
Primary Job Responsibilities:
Assessment: Functions independently to perform age-appropriate mental health assessments to patients with a psychiatric diagnosis. Provides assessments, crisis response, medication management, direct mental health services, and medical record documentation, as required.
Psychiatric Care Planning: Formulates an initial diagnosis and treatment plan for individuals with common mental health problems. Monitors the effectiveness of interventions. Provides support, psychotherapy and supportive counseling for clients in regard to psychosocial, environmental, and physical well-being. Facilitates the patient's transition within and between health care settings. Facilitates staff, patient and family decision-making by providing educational tools.
Communication and Collaboration: Demonstrates appropriate and thorough communication with all disciplines in order to facilitate/support positive patient outcomes. Collaborates with multidisciplinary team members by making appropriate referrals, providing consultation on individual patients, and at time educating staff on behavioral health topics.
Leadership: Applies leadership skills as clinical role model, leading clinical skill development, fostering critical thinking skills, promoting safe practice and acting as a liaison/consult as needed.
Committees/Participation: Actively participates and promotes professional practice by participating in applicable committees. Continues education through workshops, conferences, in-services and self-study.
Personal Evaluation: Consistently evaluates performance through self-evaluation and seeks feedback from all disciplines, recognizes when changes in practice need to be fostered to ultimately improve patient outcomes.
Minimum Qualifications: Currently licensed as Nurse Practitioner in the State of New York. Minimum of two years' experience in Psychiatric or Mental Health Center setting.

The Nurse Practitioner or Physician Assistant serves as an additional patient care provider, augmenting and extending patient services provided by the health care team. The NP/PA will work closely with members of the team and have a reasonable amount of autonomy in the care and medical decision-making for patients. A few highlights of responsibilities:
• Obtains, documents and updates medical history. Identifies health risks and needs. Performs physical examinations and conducts preventive screening procedures based on age and history. Formulates a differential diagnosis and plan of care based on history, physical examination, and clinical findings. Identifies needs of the individual, family, or community.
• Orders and review diagnostic tests. Identifies, prescribes, and manage pharmacological therapies.
• Determines effectiveness of the plan of care through documentation of patient care outcomes.
• Reassesses and modifies plan of care as necessary to achieve medical and health goals.
• Makes referrals to other health professionals and community agencies. Admits/discharges to appropriate service.
• Participates in team meetings and conferences to enhance an interdisciplinary approach to health care delivery.
• Serves on departmental and multidisciplinary committees, which influence or determine policies affecting advanced practice providers.
• Participates in and develops projects and quality improvement programs, as well as standards of care and practice.
• Maintains current level of knowledge relative to professional practice.
• Maintains all required professional documentation, such as license, certifications, infection control, ACLS/BLS, DEA as well as system requirements.
• Acts as resource to staff and implements educational programs, as needed.
Qualifications: Must have a New York State NP or PA license to be successfully credentialed to work at Saratoga Hospital.
Pay Grade: APP
Pay Range: $124K - $164K
