Job Description

Optum is seeking a Nurse Practitioner to join our HouseCalls team in Charlotte Amalie, ST. THOMAS. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

In this role you will have the ability to achieve work life balance. Flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends and no holidays required.

 

In this role, you will conduct in-home health assessments to qualified Medicare Advantage members, Community and State members and several other plans. The State-Wide Nurse Practitioner assignments will be defined based on current business needs.

 

This role requires travel approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives.

 

Primary Responsibilities:

  • Travel up to 85%
  • Conduct in-home assessments on health plan members
  • Identify diagnoses to be used in care management and active medical management
  • Formulate a list of current and past medical conditions using clinical knowledge, judgement and current findings
  • Communicate gaps in care to member’s PCP
  • Identify referral needs
  • Identify urgent and emergent situations for proper intervention
  • Educate members on topics related to health conditions and promotion
  • Compliance with all HIPAA regulations and maintain security of protected health information (PHI)

Required Qualifications:

  • FNP, GNP, ANP, board certified or eligible with an active, unencumbered licensure free any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
  • Proficient with electronic medical records and the internet 
  • Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
  • Able to travel including public transportation (eg. Trains, Airplanes, Subway, Ferry etc.)
  • Ability to travel to member’s visit location
  • Able to travel to assigned locations
  • Driver’s license and access to reliable transportation

 

Preferred Qualifications:

  • 3+ years experience in a clinical setting
  • Home Health Care or Nursing Home experience
  • Solid professional communication skills with all intradepartmental and external customers
  • Detailed oriented; dependable and reliable

 

**PLEASE NOTE** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer.  If an employee leaves Home and Community, the student loan repayments will cease.  The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
