WHO WE ARE LOOKING FOR
Franciscan Physician Network is seeking an Adult Nurse Practitioner or Physician Assistant to meet the growing demand for services in Lafayette, Indiana at our Franciscan Healthy Living Center. The provider will work in our clinic and actively engage in programs to support chronic disease management and lifestyle change programs. The APP will lead a team-based care model and provide transitional support services as well as care for weight management, tobacco cessation and several chronic illnesses. Telehealth and remote patient monitoring tools are available to support overall patient care.
WHAT WE NEED
Ideal candidate will have at least two years’ experience in primary or urgent care, strong focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with the care team to coordinate and deliver the highest quality of patient care. APP must be very team focused and have excellent leadership qualities as you will be collecting information from the group, assessing the care plan and making the final determination of the course of action.

Potential outreach to satellite office located in Crawfordsville, IN
Responsibilities include assessment, ordering medications, guideline directed medical therapies, transition from hospital to home, educating on disease process, patient outreach, and coordinating care with the entire care team

QUALIFICATIONS
Required Bachelor's Degree Nursing / Patient Care
Required Master's Degree Nurse Practitioner
3 years nursing experience in related specialty Required
Nurse Practitioner (NP) - State Licensing Board in state of practice
Basic Life Support Program (BLS) - American Heart Association
Registered Nurse (RN) - State Licensing Board
Unit Specific Certifications - Franciscan Alliance as Required by Unit
