JOB DESCRIPTION:
Join a dynamic team who provide care for our 140 + bed facility, caring for long-term residents as well as short term post-acute care patients who have been recently hospitalized. This provides a broad spectrum of clinical experience, with continuity of care that includes both chronic and acute illnesses in adults of all ages. We collaborate with an interdisciplinary team of doctors, nurses, physical therapists and care managers to provide excellent care in a rewarding environment.
Position is a 0.5 FTE. This includes with the potential for flexible hours.
Day-to-day responsibilities include admitting and discharging patients, routine periodic visits, addressing acute issues as they arise, and facilitating specialty and end-of-life care as appropriate.
Requires ability to obtain DEA certificate and VT Nurse Practitioner or Physician Assistant license.
EDUCATION:
Successful completion of PA educational program at an institution accredited by ARC-PA (Accreditation and Review Commission on Education for the PA.) Bachelor's degree required, Master's degree preferred. Current NCCPA Board Certification required and active licensure to practice as PA by the VT Board of Medical Practice.
EXPERIENCE:
Previous experience as a PA or NP desired. Geriatric experience desired.
