Acute Care Advanced Practice Provider | Swing Hospitalist

Memorial Hermann Medical Group is seeking a full-time Acute Care Nurse Practitioner or Physician Assistant with Hospitalist Experience for our Woodlands campus.

  • ACNP Certified or NCCPA Certified
  • Shift: 12PM to 12AM
  • Schedule 5 on 5 off
  • 2 years experience as an ACNP or PA
  • Preferred Hospitalist experience

JOB DESCRIPTION

Minimum Qualifications

Nurse Practitioner / Advanced Practice Nurse

Education: Graduate of a Nurse Practitioner program approved by the State of Texas; Masters of Nursing required

Licenses/Certifications:

Licensed to practice professional nursing as an Acute Care Nurse Practitioner by the State of Texas

Licensure by the State of Texas APRN and eligibility for full prescriptive authority by the State of Texas

Certified by a recognized national certification by the State Board of Texas in area of population foci and/or additional specialty recognition beyond the APRN

Eligibility for DEA licensure

Basic Cardiac Life Certification  or Advanced Cardiac Life Certification

 

Physician Assistant

Education: Graduate of a nationally accredited Physician Assistant program; Masters of Nursing strongly preferred

Licenses/Certifications:

Certified by the National Commission on Certification of Physician Assistants, Inc.

Licensed by the Texas State Board of Physician Assistant Examiners.

Eligibility for DEA licensure.

Basic Cardiac Life Certification  or Advanced Cardiac Life Certification

 

Experience / Knowledge / Skills:

Two (2) years clinical experience preferred in area of intended practice

Principal Accountabilities

Provides health promotion and the assessment, diagnosis, and management of patient problems which includes the use and prescription of pharmacologic and non-pharmacologic intervention, assessment, triage, and therapeutic management of patient/family needs throughout the disease process and across the health wellness-illness continuum as an integral part of an interdisciplinary team, according to the age/needs of the patient population served.

For prescriptive authority, APRN or PA acts under the delegation of a supervising physician, maintaining an active Prescriptive Authority agreement in accordance with regulations and policy.

Practices within approved protocols, guidelines, and clinical pathways and evidence based medicine.

Ability to float, round to multiple facilities and/or take call (first call).

Provides accurate billing, coding and documentation.

Completes medical records and submit charges in a timely manner.

Responsible for overall unit performance in collaboration with physicians or other providers.

Obtains medical histories; performs physical examinations/ reviews/orders/interprets diagnostic tests and radiographic results; formulates and manages patient treatment plan and outcomes; provides interventions and performs procedures within legal cope and authority that the APRN or PA is approved, delegated to and credentialed to perform including but not limited to incision and drainage, pelvic exams, lesion removal/destruction, suturing, and casting/splinting.

Collaborates in the development and evaluation of patient care.  Updates clinical guidelines/protocols/pathways; works autonomously and accepts accountability for outcomes; signs orders in accordance with institution and state board requirements; collaborates in research and teaching projects and applies research to practice.

Attend and actively participate in hospital multidisciplinary rounds on all patients which the hospitalist service is the primary service and when appropriate or requested when the hospitalist service is in the co-management or consultative role.

Cover Code Blue and Rapid Responses in accordance with the campus code committee’s recommendations.

Provide admission services for the Emergency Department, incoming transfer patients, and direct or emergent admissions in accordance with the hospital on call schedule and patient assignment guidelines .

Provide co-management services for subspecialty services when such services are within the scope and designation of privileges for the hospitalist Provider.

Provide consultative services for members of the Medical Staff when such services are within the scope and designation of privileges for the hospital medicine provider.

Accept transfers of care from other clinical service lines if the patient is deemed appropriate for the hospitalist service provided there is a physician to physician hand off communication related to the patient’s clinical course and the hospitalist attending accepts the patient in transfer.

Provide transfer center call for incoming non-critical care medical patients in accordance with the hospital’s on call schedule.

Provide timely evaluation of patients admitted to the hospital in accordance Medical Executive Committee bylaws.

Abide and conform to Hospital tenants, guidelines, and bylaws.

Provide internal sign out at transitions of care consistent with standards of care.

Participate in nursing and staff education when solicited by CNO, CMO, or COO.

Respond to clinical queries in a timely fashion.

Document History and Physicals within 12 hours.

Document Discharge Summaries within 48 hours of discharge order.

Maintain current understanding of the regulatory environment related to status and status patients consistent with national guidelines.

Provide post discharge coverage for appropriate home health orders for up to 2 weeks or until they have seen their primary care physicians.

Encourage in network utilization of resources for continuity of care unless specified by the patient or expertise is not available in the system. 

Respond in a timely manner to pages and call back from nursing staff unless doing so would impact patient safety or experience.

In case of emergencies or disaster provide additional coverage per campus protocol.

Participate in continuing education activities to maintain or enhance knowledge and skills.

Maintain ACLS certification.

Directs patient care throughout the health care system via Consult Orders.

Provides consultation services in collaboration with attending or primary care provider.

Communicates with the attending physician, nurse, family, and other treatment team members when appropriate, in order to obtain the appropriate patient care plan.

Effectively communicates with treatment team in a timely and professional manner to provide quality psychiatric care to patients throughout the healthcare system.

Complies with best practices and other care directives. 

Follows Performance Improvement initiatives.

Works with Memorial Hermann Physicians, Risk Managers, and Case Managers towards resolution of clinical, administrative or technical problems.

Maintains patient confidentiality and adheres to MH compliance privacy policies.

Uses interpersonal and verbal communication skills to educate patient/family on care resources; participates in process improvement and multidisciplinary clinical outcome improvement activities utilizing advanced nursing expertise.

Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
