As telemedicine continues to evolve, Advanced Practice Providers (APPs) continue to play a key role in delivering virtual care. However, with major changes to telehealth coverage and compliance set to take effect on October 1, 2025, many APPs are re-evaluating how — and where — they can continue providing care virtually.

Whether you’re currently practicing in telehealth or considering making the shift, it’s important to understand what these policy changes mean for your role moving forward, and where new opportunities are opening up.
Key Medicare Telehealth Policy Changes Taking Effect
On September 30, 2025, many of the temporary telehealth flexibilities that were extended during the COVID-19 public health emergency will officially expire. Some of the most notable changes that APPs need to be aware of include:
- Geographic and originating site restrictions return: Virtual visits for Medicare patients will again be limited to patients located in rural areas and must occur in a qualifying healthcare facility, rather than from the patient’s home.
- Audio-only services limited: Coverage for audio-only visits will be significantly reduced, impacting behavioral health and other services that previously benefited from this format.
- Cross-state practice waivers end: APPs will need to ensure they are licensed in each state where they provide care, as the federal waivers for interstate practice will no longer apply.
These changes may present logistical challenges, but they also mark the beginning of a new chapter in telehealth, one that’s more structured and built for the long term.
Where New Opportunities Are Emerging
Despite stricter federal regulations, demand for APPs in telemedicine remains strong — especially in private pay, direct-to-consumer (DTC), and employer-based models. Many organizations are building more sustainable telehealth infrastructures that don’t rely on Medicare flexibilities, creating long-term opportunities for APPs with telehealth experience.
Areas seeing continued or increased demand include:
- Mental health & behavioral services
- Chronic disease management
- Post-discharge follow-ups & transitional care
- Urgent care / on-demand platforms
Private insurers and large healthcare systems are also continuing to support virtual care as a permanent option, with many hiring APPs specifically for remote-only or hybrid roles.
Licensing, Compliance, and Credentialing Considerations
If you plan to continue offering care across state lines, you’ll need to ensure compliance with individual state licensing boards. This is particularly important for nurse practitioners and physician assistants working with multi-state organizations or platforms.
Resources such as the Interstate Medical Licensure Compact (IMLC) and Nurse Licensure Compact (NLC) may streamline this process depending on your credentials and state participation.
Additionally, many employers are requiring formal telehealth training or certifications as part of onboarding—so investing in continuing education could help you stand out in a growing field.
