Job Description

Sentara Medical Group is searching for an experienced Advanced Practice Provider (APP, such as NP or PA) to join the Sentara Senior Health Services (SHS) Home-Based Primary Care Practice. SHS is a collaboration between Sentara office-based Primary Care Providers (PCPs) and the SHS APP to bring primary care into the homes of patients with chronic medical conditions, those who have difficulty accessing primary care due to mobility and/or transportation barriers, and those who would benefit from more frequent engagement with primary care to maintain medical stability and/or maximize their outcomes.

The SHS APP manages chronic, acute, and transition of care medical needs during the primary care home visits. Each home visit includes obtaining a thorough history and exam that drives the medical decision making and assessment and plan, medication management, ordering of labs or imaging, and referring to specialty practices or other Sentara resources, as indicated. In addition, care gaps are identified and closed, such as the completion of the Medicare Annual Wellness Visit and HCC coding opportunities. As the SHS practice is a collaborative practice model with the referring office-based PCPs, excellent communication skills are required.

Position Highlights:
The SHS APP provides care to patients within the Chesapeake area, within a 30-mile radius of Sentara Leigh Hospital
Candidates must have the ability to provide high quality, comprehensive primary care with minimum supervision
Experience with technology is essential, including EMR (preferably Epic), ability to utilize and troubleshoot a laptop and Wi-Fi Hotspot, and use of clinical applications on smart phone while conducting home visits
Full time, 4 clinical days/week with 1 administrative day (1.0 FTE)
100% remote (not office based) with all primary care visits conducted in patients homes and 1 telehealth appointment / clinical day

Minimum Requirements:
At least 3 years of APP experience in a primary care setting, such as Internal Medicine, Family Medicine, or Geriatrics
Graduation from an accredited APP program
Professional Licensure, National Certification, and current DEA license
Chronic disease management experience highly preferred
