Mendota Health is hiring Nurse Practitioners and Physician Assistants to provide mobile wound care to patients in their homes across North/Inland Connecticut. This role is ideal for an APP who values high-touch, relationship-based care, with visits lasting up to 60 minutes and a focus on treatments that make a meaningful difference in patients’ lives.

Role & Schedule
- Employment Type: Full-time, salaried
- Schedule: Monday–Friday
- No on-call
- No mandatory weekends, evenings, or holidays
- Setting: Patients’ homes within North/Inland Connecticut (Hartford, Somers, Enfield, Thompson, Marlborough); daily local travel with mileage reimbursed
- Support: All equipment, medical supplies, and training provided
In this role, you will provide advanced wound care as an experienced member of the healthcare team, managing patients from assessment through follow-up.
Key Responsibilities
- Provide advanced wound care to patients in their home setting.
- Conduct comprehensive assessments and identify factors that increase the risk of wound formation.
- Develop and implement individualized care plans, including coordination of care between visits.
- Perform advanced treatments such as surgical debridement and CAMP.
- Manage medications: order necessary medications and give clear instructions for use.
- Educate nursing staff and maintain open communication with primary care physicians, families, and clinical teams.
- Support practice operations and policies that promote access to care.
- Work at the top of your scope of practice, using strong clinical judgment.
- Provide patient education and proactive disease management.
- Adapt care approaches to the developmental level and needs of the patient population.
- Demonstrate a consistent commitment to high-quality, reliable patient care.
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- At least one year of experience as an Advanced Practice Provider
- Clinical background in one or more of the following: adult, family, geriatric, podiatry, or vascular specialties
- Board Certified/Prepared through AANP, ANCC, or NCCPA
- Wound care experience required
- NP national certification as ANP, FNP, or GNP (or ability to obtain)
- Current, unrestricted license in the state of assignment (or ability to obtain)
- Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority, where applicable
