Job Summary

The Mendota Health Advanced Practice Provider (APP) has an important role within Mendota Health. The APP provides mobile advanced wound care while acting within their professional boundaries. The APP will provide care to patients in multiple settings including (but not limited to) independent living, assisted living, and/or long-term care facilities. The APP will provide care for the patient starting from initial history intake to clinical assessment, diagnosis, treatment, and evaluation. The APP will work collaboratively with multiple members of the team to deliver strategic priorities while ensuring quality wound care and enhancing patient outcomes.

Education, Experience, Training

Consistent with the appropriate state credentialling policy, the APP must meet the following criteria:

Licensure:

  • Valid current and unrestricted state licensure as a Registered Nurse and an Advanced Practice Registered Nurse, or Physician Assistant.
  • Certification as a Nurse Practitioner or Clinical Nurse Specialist by an approved certifying body (AANP, ANCC, or NCCPA).
  • Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required.

Educational Background:

Advanced degree (Master’s or Doctorate) prepared as a Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant.

Prescriptive Authority:

  • State licensure for prescriptive authority.
  • Current, or eligibility for, a DEA license in the state of practice.

Certifications:

  • Current Basic Life Support (BLS) certification.
  • Current Advanced Cardiovascular Life Support (ACLS) certification (preferred, not required)
  • Wound care certification (CWS, CWCN, CWOCN) (preferred, not required).

Experience and Skills:

  • Clinical experience of at least one year as an APP is preferred.
  • Clinical experience in wound care is preferred.
  • Proficiency in wound debridement is preferred.
  • Proficiency in skin substitute use is preferred.
  • Experience with current wound care treatment options is preferred.
  • Written and verbal communication skills.
  • Computer proficiency (Microsoft Office Suite or related software preferred).
  • Exceptional customer service orientation projecting empathetic, compassionate, and professional demeanor with each interaction.
  • Ability to work independently.
  • Ability to manage and complete multiple daily tasks.
  • Ability to effectively manage time and priorities.
  • Excellent communication and interpersonal skills.

Performance Expectations:

The APP acting within their professional boundaries will provide care for the patient using evidence-based medicine and quality care. The authority, responsibility, and accountability of the APP are as follows:

Clinical Responsibilities:

Independently manages patients referred to Mendota Health for evaluation and treatment of non-healing, chronic wounds, including patient education, diagnostic testing, review of diagnostic results, collaboration with the Medical Director and other specialists, facilitation of appropriate referrals, and coordination of clinical follow-up.

Deliver a high standard of patient care using advanced clinical skills and a broad and in-depth theoretical knowledge base.

Prescribes appropriate pharmacologic and non-pharmacologic treatments.

Manage a clinical caseload as defined by leadership, addressing the needs of patients in the mobile wound clinic setting.

Adequately prepares for visits by reviewing the medical record and ensuring adequate supplies and equipment are available for timely completion of medical records.

Educational Responsibilities:

  • Teach patients and/or caregivers’ information and skills related to their care.
  • Maintain professional competencies through ongoing education.
  • Support organization education and collaboration through activities such as precepting, in field support to community engagement directors (CEDs) and assisting in organizational educational projects.

Communication Responsibilities:

Communicates in a professional, positive, and appropriate manner with all stakeholders including patients, families, care team members, and Mendota Health team members.

Ensure effective communication with patients, families, care team members, and physicians regarding patient progress, plan of care, and treatment plan.

Provide a hand off including updates to treatment plan to home health, assisted living, or other partners on the same day of the visit.

Informs the patient, caregiver, and/or staff as well as the Mendota Health team if there are changes to the schedule.

Ensure effective and timely communication in an appropriate manner regarding any concerns to the Director of Clinical management.

Staff Development Responsibilities:

  • Participate in the orientation of new employees.
  • Work in partnership with team members to provide high-quality care.

Quality and Improvement Responsibilities:

  • Implement care plans and clinical pathways to ensure progression toward quality outcomes.
  • Participate in developing and monitoring action plans to ensure performance improvement and regulatory compliance.
  • Strive to perform tasks correctly the first time, using quality tools and techniques as appropriate.
  • Demonstrate flexibility and resilience in response to change.
  • Adheres to all Mendota Health wound care guidelines and documentation requirements including completion of medical records within 24 hours of visit and submission of patient care orders on same calendar day of visit.

Collaboration and Teamwork Responsibilities:

  • Collaborate with physicians and team members regarding changes in patient conditions and revise care plans based on team input.
  • Support the roles of all co-workers and act as a resource for the interdisciplinary team regarding coordination of care, pathways, and discharge planning.
  • Maintain effective relationships with primary providers, specialists, and home health agencies.
  • Participate actively as a team member on both market and organizational levels, fostering collaboration and recognizing the efforts and successes of others.
  • Participates in weekly roundtable and weekly meetings with Director of Clinical Management.
  • Consistently attends weekly market meetings.

Leadership and Organizational Engagement:

  • Provides wound care in accordance with Mendota Health policies, state regulations, and federal regulations.

  • Complete all required company training/s assigned by role.
  • Attending Mendota Health All Staff Meetings.

Work Environment:

**This job is 'on the road' traveling LTCs & Patient Facilities around Gary, IN territory/market.

  • Ability to travel within assigned market.
  • Ability to travel out of state if warranted.
  • Prolonged periods of sitting at a desk and working on a computer.
  • At times, may be required to lift up to 15 pounds.
