Job Description

About this role: 

As an Advanced Practice Provider on our Post-Acute Care Team, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience.  In this role you will have some remote-from-home work but primarily will see Post Acute Care - Skilled Nursing patients at the facilities we serve in Salt Lake County. 

  • This position includes up $7,500.00 toward eligible relocation assistance if relocating to Salt Lake City Utah Area 

  • You will practice with Intermountain Health at the facilities we serve in the Greater Salt Lake County   

  • Mileage Reimbursement available for driving between facilities  

  • Your hours will be Monday through Friday, 8am-5pm- to the facilities you are assigned to 

  • You will be on call 1:9 rotation for days and weekend

What you’ll bring: 

  • Reliable vehicle and active unobstructed Utah state driver's license required 

  • APRN or PA-C degree from an accredited program 

  • Training or experience in geriatrics medicine is preferred but not required 

  • New Graduates will be considered 

  • Board certification or eligibility in the designated discipline 

  • Active, unobstructed Utah medical license, or the ability to obtain one 

  • Full prescriptive authority and an active DEA, or the ability to obtain one 

  • Ability to successfully complete Intermountain Health’s credentialing process
