About this role:
As an Advanced Practice Provider on our Post-Acute Care Team, you will work with a team of healthcare professionals committed to providing quality care and excellent patient experience. In this role you will have some remote-from-home work but primarily will see Post Acute Care - Skilled Nursing patients at the facilities we serve in Salt Lake County.
This position includes up $7,500.00 toward eligible relocation assistance if relocating to Salt Lake City Utah Area
You will practice with Intermountain Health at the facilities we serve in the Greater Salt Lake County
Mileage Reimbursement available for driving between facilities
Your hours will be Monday through Friday, 8am-5pm- to the facilities you are assigned to
You will be on call 1:9 rotation for days and weekend

What you’ll bring:
Reliable vehicle and active unobstructed Utah state driver's license required
APRN or PA-C degree from an accredited program
Training or experience in geriatrics medicine is preferred but not required
New Graduates will be considered
Board certification or eligibility in the designated discipline
Active, unobstructed Utah medical license, or the ability to obtain one
Full prescriptive authority and an active DEA, or the ability to obtain one
Ability to successfully complete Intermountain Health’s credentialing process
