Summary

Join CentraCare’s Melrose Clinic as a Family Medicine Advanced Practice Provider (NP or PA) and build your own patient panel while delivering comprehensive care to patients of all ages. This role offers the opportunity to practice independently in a supportive rural clinic environment, managing acute and chronic conditions, providing preventive care, and coordinating complex care with robust clinical, community, and specialty support resources.

 

Schedule

  • Full-time | 9–1.0 FTE | Onsite – CentraCare – Melrose, MN
  • Day shifts | 8am-5pm
    • Option for a four-day work week or five- day work week
  • 1:6 Saturday Clinic Rotation | No call | No holidays

 

Pay and Benefits

  • Salary range:  $109,241.60 - $124,904 per year
    • Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
    • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
    • May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.

Qualifications

Required:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Eligibility for Minnesota licensure as a Nurse Practitioner or Physician Assistant
  • Current AHA BLS certification
  • DEA registration (or ability to obtain prior to start)

Preferred:

Prior experience in Family Medicine, Emergency Medicine, or Urgent Care
