Hospice Care Advanced Practitioner

Northern Light Home Care and Hospice in South Portland, Maine, is seeking a full-time, compassionate, and skilled nurse practitioner or physician associate to join our dedicated hospice care team.

In this rewarding role, you will review hospice eligibility, assess patient charts, and provide clinical guidance to nursing staff who perform community-based home visits.  While this position is primarily remote, there may be occasional opportunities for face-to-face visits.  

You’ll support high-quality care for patients across multiple facilities in the greater Portland area, and the right candidate may reside anywhere in the southern, Central or the Maine Highlands region.  Your clinical expertise and heartfelt dedication to patients and their families will make a meaningful difference in their lives during times of greatest need.
