Family Medicine APP – Litchfield, MN

Summary
Join CentraCare as a Family Medicine Advanced Practice Provider (NP or PA) at CentraCare – Litchfield Clinic, a well-established practice supported by a collaborative care team and a new clinic space currently under development. In this role, you will provide comprehensive, patient-centered care to an established panel of patients across the lifespan, including acute and chronic disease management, preventive services, routine checkups, screenings, immunizations, and personalized lifestyle counseling. This is an excellent opportunity to practice independently while benefiting from the resources, specialty access, and integrated support of one of Minnesota’s leading rural health systems.

Schedule

• Full-time | 1.0 FTE | Onsite at CentraCare – Litchfield 
• Days | 8-hour shifts | 7:30am – 5pm
• Monday – Friday, no weekends, no call, no holidays

Pay and Benefits

• Salary range: $109,241.60 - $124,904 per year
  • Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
  • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
  • May be eligible for additional pay through bonuses, quality incentives, or production-based (RVU) compensation.

Qualifications
Required:

• Current NP certification or Minnesota Physician Assistant licensure (or eligibility)
• Current American Heart Association BLS Healthcare Provider card
• DEA registration prior to receiving clinical privilege
