Summary
Join CentraCare as a Family Medicine Advanced Practice Provider (NP or PA) at CentraCare – Litchfield Clinic, a well-established practice supported by a collaborative care team and a new clinic space currently under development. In this role, you will provide comprehensive, patient-centered care to an established panel of patients across the lifespan, including acute and chronic disease management, preventive services, routine checkups, screenings, immunizations, and personalized lifestyle counseling.
