Expected pay for this position is $55.20 - $63.65/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
OSF OnCall Mobile Community Care
OSF Mobile Community Care is hitting the road with a new Primary Care Mobile Vehicle. We are seeking a dynamic, self-motivated, independent provider to reach patients where they are, utilizing in person and virtual care supporting our rural communities. This experienced provider will engage/interact with the various populations especially trying to reach those underserved or have limited access to healthcare. This opportunity spans in Central, East, and Northcentral, Illinois. While the expected care demographic will be adult, a familiarity with pediatric and infant care is encouraged. We are seeking an innovative thinker with the ability to see the big picture and meet patients where they are on their healthcare journey. Our ideal provider will connect with patients in a way that drives better patient engagement, promotes commitment to continuous improvement, and promotes our core team values. In addition to providing care, this APP will help support operations and partner with community leaders on the benefits of mobile community services.

REQUIRED QUALIFICATIONS: Education: Master’s degree in advanced practice nursing . Experience: Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients serve. Licensure/ Certification: IL Advanced Prac Nurse IL Reg Nurse Fed Controlled Sub Lic BLS/ACLS Certified Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Education: N/A Experience: Experience with EPIC electronic health record. 1-2 years of experience as an Advanced Practice Nurse Licensure/ Certification: N/A Other Skills/ Knowledge: N/A
