OnePeak Medical is expanding into one of Oregon’s most desirable regions and seeking a Primary Care Functional Medicine Nurse Practitioner or Physician Assistant to join our Bend clinic. This role allows you to deliver holistic, root-cause-focused primary care in a supportive, integrative, and growing practice—while enjoying an exceptional work–life balance.

Responsibilities & Duties

• Deliver direct patient care through office visits, telehealth, and same-day appointments
• Conduct comprehensive assessments, order and interpret labs/imaging, diagnose acute & chronic conditions
• Create personalized treatment plans including lifestyle medicine, supplements, hormone therapies, and prescriptions
• Educate and empower patients through preventive health and functional medicine
• Collaborate closely with health coaches, nutritionists, mental health providers, and clinical support staff
• Document all encounters in NextGen EHR timely and accurately
• Participate in team meetings, quality initiatives, and ongoing training
• Support a positive, team-centered clinical environment

Required Qualifications

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or related specialty
• Minimum 2 years of clinical experience preferred
• Genuine interest in preventive, functional, and lifestyle medicine
• Strong communication and patient engagement skills
• Ability to thrive in a collaborative, fast-paced setting
• Proficiency with EHRs (NextGen preferred)
