OnePeak Medical is seeking a Primary Care Functional Medicine Nurse Practitioner or Physician Assistant to join our Eugene–Coburg clinic. Responsibilities & Duties

• Provide direct patient care via office visits, telehealth, and occasional same-day appointments
• Conduct comprehensive assessments, order and interpret labs/imaging, and diagnose acute & chronic conditions
• Develop personalized treatment plans including lifestyle modification, supplements, hormone therapy, and prescriptions
• Educate and empower patients using preventive health and functional medicine principles
• Collaborate closely with health coaches, nutritionists, mental health providers, and support staff
• Document patient encounters in NextGen EHR
• Participate in team meetings, quality improvement initiatives, and functional medicine training
• Contribute to a positive, team-oriented environment

Required Qualifications

• Active Oregon license as a Nurse Practitioner (FNP, AGNP, AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or related specialty
• Minimum 2 years of clinical experience preferred
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship-building skills
• Comfortable working in a fast-paced, collaborative setting
• Proficiency with EHRs (NextGen preferred)
