The role of the Advanced Practice Provider (APP) is to contribute to the clinical, educational, and research missions of the organization. May provide clinical care in the inpatient or outpatient setting, telehealth, and triage.  Provides regular health assessments, diagnosis, and management of acute and chronic conditions and formulate plans that include treatment, psychosocial support, health education, and counseling. In addition, the role will collaborate with other health professionals in assessing and managing health problems, maintaining continuity of care, and coordinating community services. Contributes to the development, maintenance, evaluation, and change of the health care delivery system.

 

 

Responsibilities

  • Provides care for adult patients in the Department of Medicine electrophysiology clinic.
  • Performs new and follow-up visits gathering data from initial evaluation, develops and documents an assessment and plan of care.
  • Generates progress notes with each outpatient visit.  Keeps problem sheets up to date.  Maintains communication with referring physicians and other health care providers.
  • Coordinate care with cardiac device clinic, including coverage of device clinic when needed.
  • Is expert resource for providers and patients regarding electrophysiology questions or issues.
  • Generates and executes evaluation and management plan in urgent circumstances.
  • Perform clinic-based procedures, including implantation of implantable loop recorders and cardioversion.

 

Essential Duties

  1. Provides quality care for patients and families, manages chronic and acute conditions, and manages health maintenance and promotion.
  2. Performs and documents the history and physical exam, gathers data from the medical record, reviews outside medical records, communicates with referring providers, and develops and implements the plan of care.
  3. Monitors and documents the progress of each patient, revises plan of care as needed, interprets and monitors test results. Identifies and appropriately intervenes with problems that require specialty referral or escalation to a higher level of care.
  4. Acts as a consultant and presents current educational programs to requesting community groups, organizations, or individuals.
  5. Contributes to the education and research mission of the organization through participation in research or quality improvement projects, presentations, professional organizations, precepting, and other scholarly activities. 
  6. Functions as a role model and encourages professional behaviors in dealing with patients, families, significant others, and other members of the multidisciplinary team. 
  7. Represents advanced practice, Department, or Division on appropriate organizational committees.
  8. Abides by applicable National Jewish policies, and complies with all laws, rules, and regulations concerning advanced practice, including but not limited to Colorado Medical Board, Joint Commission Medicare and Medicaid rules and regulations. 
  9. Maintains current license, board certification, BLS and/or ACLS or PALS certification appropriate to clinical responsibilities and stays up to date with changes in regulations.

Minimum Qualifications

  1. Education: Graduate degree from an accredited Physician Assistant or Nurse Practitioner program required.
  2. Work Experience: 1 year of recent and related experience as an Advanced Practice Provider preferred. Experience in Cardiology and Cardiac Electrophysiology preferred.
  3. Special Training, Certification or Licensure: Colorado PA license or APRN license and prescriptive authority. DEA Registration. PA or NP certification by a national certifying body. Current CPR/BLS certification is required. ACLS or PALS certification if applicable for specific practice area.
  4. Certified Cardiac Device Specialist (CCDS) Certification preferred.
