Helen Porter Part-Time Nurse Practitioner or Physician Assistant Opportunity!

Position Summary

We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Helen Porter, a 100-bed facility specializing in long-term care and sub-acute rehabilitation. This part-time role offers a consistent schedule with opportunities for additional shifts and potential growth to full-time, based on patient census and care needs.

Key Responsibilities

  • Provide high-quality care to a diverse patient population, primarily long-term care residents with some rehabilitation patients.
  • Collaborate with a multidisciplinary team, including physicians, nursing staff, administrative leaders, the therapy team (PT, OT, SLP), and social workers, to support comprehensive patient care.
  • Conduct patient assessments, develop care plans, and manage ongoing treatment.
  • Document care using Epic EMR (training provided).
  • Maintain compliance with facility protocols and regulatory standards.

Schedule & Coverage

  • Standard Schedule: 2 set days per week (Wednesday and Friday preferred).
  • Shift Length: 10–8-hour shifts.
  • Coverage Area: 100-bed facility (primarily long-term care).
  • Team Structure: 2 physicians + 1 full-time APP (typically 2 providers onsite daily).

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Vermont.
  • Experience in long-term care, geriatrics, or rehabilitation preferred.
  • Strong communication and collaboration skills.
  • Ability to work independently and as part of a team.
  • Geriatric experience preferred; however, new graduates are welcome to apply.

 

Compensation: $52.33/hr – $73.27/hr
