NP/PA – Primary Care, High-Risk Population

Pinehurst Medical Clinic (PMC) is seeking a compassionate and clinically skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care team serving a higher-risk patient population. This role supports PMC’s mission to provide high-quality, patient-centered, and compassionate care that improves the health and well-being of the communities we serve.

 

This position is ideal for a provider who values relationship-based care, proactive outreach, and frequent patient engagement to improve outcomes for patients with complex medical and social needs. You will work closely with physicians, nurses, care managers, and behavioral health partners to deliver comprehensive, coordinated, and longitudinal care across the continuum.

 

Key Responsibilities

  • Provide comprehensive, evidence-based primary care services to a panel of higher-acuity patients, including those with multiple chronic conditions
  • Deliver increased care touchpoints through in-person visits, telehealth, follow-ups, and proactive care coordination
  • Partner with patients and families to develop individualized care plans aligned with patient goals and clinical best practices
  • Collaborate with interdisciplinary care teams (RN care managers, pharmacists, social workers, behavioral health) to address medical, behavioral, and social needs
  • Close preventive and chronic care gaps through population health and quality initiatives
  • Support safe and effective care transitions following hospitalizations or emergency department visits
  • Support patients with advanced care planning
  • Document care accurately and efficiently within the electronic health record
  • Contribute to continuous quality improvement and value-based care initiatives

 

Ideal Candidate

  • Certified Nurse Practitioner (NP) or Physician Assistant (PA) with an active state license
  • Experience in primary care, internal medicine, geriatrics, palliative care or chronic disease management preferred
  • Demonstrated commitment to patient-centered, high-quality care
  • Comfort working with complex patients requiring frequent engagement and follow-up
  • Strong communication, collaboration, and patient education skills
  • Experience in value-based care, population health, or team-based models is a plus
