Home Health Nurse Practitioner Southern California | $150K+ Compensation | No Nights, Weekends

Were seeking a compassionate and skilled Nurse Practitioner (NP) to join a leading value-based care organization in Southern California. This role provides in-home patient visits allowing you to deliver truly comprehensive, patient-centered care to those that are homebound.

• Base Salary: $135K$152K (commensurate with experience)

Your Role

• Conduct health assessments and preventive care visits within established clinical guidelines

• Manage acute and chronic conditions through evidence-based treatment plans

• Collaborate closely with physicians and care teams to develop coordinated care plans

• Order, interpret, and evaluate diagnostic tests to guide care decisions

• Prescribe medications and therapies in accordance with scope and regulations

• Provide education and empowerment to help patients proactively manage their health

Requirements

• Masters degree in Nursing (MSN) required

• Minimum of 1 year of experience as a Nurse Practitioner

• Current and unrestricted RN and NP licenses in California

• DEA license preferred

• Passion for patient-centered, value-based healthcare
