Primary Care Nurse Practitioner Cleveland, OH

COMPENSATION: Salary $140k + $10k Bonus

Our client is a physician-led, mission-driven primary care organization dedicated to transforming healthcare for seniors through high-quality, value-based care. To support continued growth, we are seeking high-caliber primary care providers and clinical leaders who are motivated by purpose, professional development, and the opportunity to make a meaningful impact in the lives of patients and communities.

Role:
Compensation: $140k
Bonus: Up to 10k year 1 and goes up as a partner.
18-20 patients a day
Hours: Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15)

Small Patient Panel 450 patients max
Administrative time included in schedule
No nights or weekends
100% outpatient

Dedicated Care and support team (have your own MA, Scheduler & Referal Speecialist
Patient Population: Seniors with 3-5 chronic diseases

Requirements:

Board Certified Nurse Practitioner

2 + years NP outpatient primary care experience

Geriatric experience (preferred)
